Ohioans is currently looking for a passionate Medical Social Worker to join our team!  

The Medical Social Worker for Secondary Case Management will implement the standards of care for medical social work services and collaborate with primary case managers to assess, plan, facilitate, and advocate options and services to most appropriately meet individual patient's needs.  Secondary Case management will assist primary case managers to improve patient's overall knowledge and compliance of their care plan.

Job Requirements

  • Must have a Masters or Doctorate degree accredited by the Council on Social Work Education.
  • Must be licensed in Ohio and willing to get license in Michigan 
  • Minimum of one (1) year's experience in health care
  • Experience in home health preferred
  • Demonstrates good verbal and written communication and organizational skills.
  • Possesses and maintains current CPR certification.
